Juan and Heidi bring over 40 years of combined experience working within hospitals, skilled nursing communities, hospice support services, and senior care systems. Juan has spent more than 20 years guiding families through important care decisions and helping loved ones transition into their next phase of life.

Heidi supports families and works closely with hospitals and skilled nursing facilities to help ensure a smooth transition into assisted living, memory care, or other appropriate levels of care.
